Plan A1 $1695

Picture
  1. Transfer of deceased from place of death within 40 km of Medicine Hat city limits.
  2. Preparation of documents to obtain permits for burial or cremation.
  3. Grey cloth covered, flat top casket with white satin interior for burial.
  4. Suitable container when cremation is selected.
  5. Use of chapel for funeral service or equipment and staff required for service at a church.
  6. Ushers for funeral service
  7. Taped music for funeral service
  8. Basic stationery package (memorial book & service folders).
  9. Funeral coach to Medicine Hat cemetery or crematorium.
  10. 2.5 hours consulation time
  11. Complimentary Christmas Candlelight Ceremony/Banquet
  12. One year website memorial tribute
  13. Two (2) copies of Funeral Director's Statement of Death 
  14. Memorial Society Administration fee extra cost - $25

  8. Ushers for funeral service.
  9. Taped music for funeral service.
  10. Basic stationery package (memorial book & service folders).
  11. Two (2) copies of Funeral Director's Statement of Death

Plan B1 $1245

Plan B2 $1545

Plan C

Any deviation from plans A1, A2, A3, B1, or B2 (other than the listed additional costs) will void those plans. CCMS members however are entitled to a 10% discount on casket, urn and professional service fees. 

Notes:

  • Options do not include cemetery charges. i.e. Purchase of the plot, opening and closing of the grave or cremation urns.
  • Transportation of remains under all Memorial Society Plans will be provided to Medicine Hat cemetery or or crematorium.
  • You are eligible to lock-in the rate at today's prices by prepaying any of these plans with Cook Southland Funeral Chapel. Please phone for an appointment.

Additional fees:
  • After hours arrangements (5 p.m. to 8 a.m.) - $175
  • After hours transfers (5:00 p.m. to 8:00 a.m.)  $175
  • Weekend arrangements - $325
  • Overtime fee for ceremony on Saturday, Sunday or holidays  $425.00
  • Additional consultation time  $65 per hour
  • Transfer from a destination that requires more than one transfer personnel  $140 
  • Transport of deceased outside 40km from Medicine Hat city limits  $2.25 per km per vehicle
  • Additional copies of Funeral Director's Statement of Death $25. each (if more than 2 are required)
  • Certificate of cremation $25
  • Preparation and delivery of obituaries to local newspapers $75. for first and $10. for each additional
  • Obituary notice in Medicine Hat area Newspapers at cost
  • Selection of urn (less 10% discount)
  • Use of urn arch (carrying device)
  • Graveside service with urn/casket
  • Staff for prayer service at church or chapel
  • Use of reception room, including hostess, coffee tea and juice
  • Limousine and driver
