What is our member registry?

It is a repository of member' contact information, funeral preferences and vital statistics information. This information is needed by your next-of-kin, your executor and the funeral home in order to plan your last wishes and government paperwork at time of death. It is password protected and conforms to our Privacy Policy and Government of Alberta regulations.  Our funeral service providers can access your profile at time of need.

What is A member Profile?

Your member profile, is your guidance to your next-of-kin and the funeral home after you pass away. This information will only be accessed by our Service Providers responsible for your end of life care. The information is collected for three different purposes:
  1. To convey your funeral preferences and any plans in place. 
  2. To assist the funeral home and your next-of-kin complete the certificate of death and the form for Vital Statistics. 
  3. To assist with compiling information for your obituary.

Accessing Your profile

WHAT HAPPENS IF I DON'T HAVE AN EMAIL ADDRESS?

If you provided an email address to us upon signing up, you will have been emailed login instructions. If you did not provide us with your personal email address, and you would like to access your profile, you will need to contact our Membership Services Coordinator and provide her with your email address or use our online form. You can be a member without an email address but you won't be able to add you funeral preferences information to your profile.

To access your profile, you must have your own email address. It becomes your individualized identifier (like a user ID). After you login to your profile page, you will be able to see a VIEW PROFILE/EDIT PROFILE button. Click on the button and you will be directed to another page with a list of questions. Click on the edit button to edit each field.

What do I tell my family about accessing my profile at time-of-need?

It is important to let your family know that you have a CMS membership and that there are special priced funeral plans for members. They need to know they can save hundreds if not thousands of dollars of your funeral.  When you pass away, the family must contact any one of our funeral partners in Alberta. Prices differ by funeral home so you may be able to save more money by reviewing the different contracts (listed under each partner). Even funeral partners outside your city or town may be able to provide service to you with an extra fee for mileage.

Funeral  Home Access to Your Registry

Our partner funeral homes have the ability to access your profile at time-of-need. Bound by the funeral code of ethics, your information will be shared with your next-of-kin for the purpose of planning your funeral arrangements. The more information you provide in your profile, the easier it is for those involved to make necessary decisions, while ensuring your wishes are fulfilled. 

As part of the process, the Government of Alberta requires background information about the member for Vital Statistics. This includes a social insurance number (SIN) and other information. For your protection, we do not collect a SIN in the member profile.
