These forms are for use by members who have not entered their information online. Be sure to give a copy to your next of kin, because the service provider will not have a copy at the funeral home. If you entered your information online, you may print a copy to keep by pressing Ctrl+P on your keyboard.

Guidance Form
This form is used to provide your next of kin with your service preferences and vital statistics information. This form should be taken to the funeral home when making initial funeral arrangements. (2 formats provided: Word Document, PDF). It is a good idea to provide copies of this form to your executor and next of kin. |

Membership Card
Although the membership card is not required to verify membership in CCMS, it serves as a useful reminder to your next of kin, that you are a member and entitled to the services therein. We recommend you place a copy with your paperwork and give a copy to your next of kin. |
