Central Alberta Service Provider' Contract
Effective February 15,  2022 - February 15, 2024

Please note: Prices include the cost of services only.  Products required or requested (for example, casket, urn, Funeral Director’s Statement of Death, etc.) will be an additional charge.
BURIAL PLANS

Plan A1 – Basic Disposition/Burial with Funeral Service (Body not Present)                             $3650
Includes:
· Arrangement conference at the Service Provider’s facility with a
licensed funeral director (60-minute limit)
· Transfer of the deceased to the Service Provider’s facility from a
facility with a climate-controlled morgue (with Service Provider’s
Municipality)
· Provision of documents to obtain Disposition Permit from Alberta
Vital Statistics
· Graveside Service with casket (cost of casket is an additional charge)
· Usher to assist at funeral service (within the Service Provider’s municipality)
 
 Plan A2 – Funeral Service with Burial to Follow (Body Present)                                                    $4595
Includes:
· Arrangement conference at the Service Provider’s facility with a licensed funeral director (60-minute limit)
· Transfer of the deceased to the Service Provider’s facility from a facility with a climate-controlled morgue (with Service Provider’s Municipality)
· Provision of documents to obtain Disposition Permit from Alberta Vital Statistics
· Usher to assist at funeral service (within the Service Provider’s municipality)
· Transfer of the deceased to the cemetery (within the Service Provider’s municipality
· Funeral Coach and attendant to assist with the Graveside Service
· Body present at Funeral Service with Casket (cost of casket is an additional charge)

CREMATION PLANS

Plan B1 – Basic Cremation                                                                                                                            $1950
Includes:
· Arrangement conference at the Service Provider’s facility with a licensed funeral director (30-minute limit)
· Transfer of the deceased to the Service Provider’s facility from a facility with a climate-controlled morgue (with Service Provider’s Municipality)
· Provision of documents to obtain Disposition Permit from Alberta Vital Statistics
· Cremation Process
 
Plan B2 – Basic Disposition/Cremation with Funeral Service (Body not Present)                   $3650
Includes:

Plan B3 – Funeral Service with Cremation to Follow (Body Present)                                           $4995
Includes:
· Arrangement conference at the Service Provider’s facility with a licensed funeral director (60-minute limit)
· Transfer of the deceased to the Service Provider’s facility from a facility with a climate-controlled morgue (with Service Provider’s Municipality)
· Provision of documents to obtain Disposition Permit from Alberta Vital Statistics
· Cremation Process
· Usher to assist at the Funeral Service (within the Service Provider’s municipality)
· Body present at Funeral Service with casket (cost of casket is an additional charge)

PLAN C - (may not be combined with any other Memorial Society plan)
·  10% discount on specific services offered by the Service Provider
·  10% discount on specific caskets offered by the Service Provider
·  10% discount on specific urns offered by the Service Provider
 
Any services described above, if provided outside the Service Provider’s municipality, outside the hours of 9:00 am. and 5:00 p.m. on Monday through Friday, or on statutory or civic holidays will be subject to an additional charge.
